Dear Parents,
As the time comes closer for all students to return to school, it’s important that we take all the precautions we can to keep our community safe. Yesterday, we received notification from the KHDA to distribute health declaration forms to all families. Attached to this email is a declaration form to be signed by all parents who have chosen face-to-face/blended learning for their children (this includes the full on campus program for KG).
The completion of this form is mandatory as per KHDA which asks parents to confirm that their children are healthy at the time they start school, and that they do not have any chronic medical conditions that will be aggravated in case of Covid-19 infection.
Those who have opted for 100% remote learning do not have to complete the form. However, please note that if circumstances change which warrants that your child be on campus, the form must be completed.
The form is a word document and available in both English and Arabic (see attached). In order to adhere to paperless guidelines, you can complete the form in the following ways:
Print the form, complete and sign it. Then scan (or take a photo) and email it to your child’s relevant division. (Please save the file with your child’s name and grade).
Complete the form digitally and include an e-signature. Save the document. Then email it to your child’s relevant division.(Please save the file with your child’s name and grade)
IMPORTANT: Please complete one form per child and email them to the relevant school division using the following email addresses:
1) ELEMENTARY – EShealthdecform_daa@gemsdaa.net
2) MIDDLE SCHOOL – MShealthdecform_daa@gemsdaa.net
3) HIGH SCHOOL – HShealthdecform_daa@gemsdaa.net
Yesterday, we also sent you a Travel Declaration Form required by the Dubai Health Authority.
We understand that with the changing environment we are in, there is much information coming your way. We do however want to impress upon you, the urgency and importance of completing the mandatory declarations of both the KHDA and DHA in order for your children to begin school on Sunday.
